It should test a decision
Strong practice items ask the learner to choose the best action or concept for a scenario. Pure memorization questions are sometimes useful, but they should not dominate the bank.
Distractors should be plausible
Wrong choices should reveal common misunderstandings. If every wrong answer is obviously absurd, the item does not prepare the learner for real exam pressure.
Explanations should teach
A useful explanation identifies the correct concept and explains why tempting alternatives do not fit. Objective mapping makes it easier to target follow-up study.
